For New Patients
For initial appointments, you should arrive at the clinic 15 minutes prior to your scheduled appointment time to complete a New Patient information sheet. If you wish, you may save that 15 minutes by printing the form and bringing the completed form with you.
This form provides valuable information for the physician and will enable us to establish a history and administrative file for you.
Referrals
If your insurance company requires a referral from his or her primary care physician to see one of our specialists, it is the your responsibility to provide us with all required referral documentation on your initial appointment.
Patient Privacy
Thomasville Orthopedic Center respects the privacy of its patients and follows federal guidelines in protecting your health information.
In 1996, the U.S. Department of Health and Human Services issued the Privacy Rule also referred to as HIPPA Privacy Rule. A major goal of the Privacy Rule is to assure that individuals’ health information is properly protected while allowing the flow of health information needed to provide and promote high quality health care and to protect the public's health and well being.
The privacy rules protects all “Individually identifiable health information” including demographic data, that relates to:
- the individual’s past, present or future physical or mental health or condition,
- the provision of health care to the individual, or
- the past, present, or future payment for the provision of health care to the individual,
Individually identifiable health information includes many common identifiers (e.g., name, address, birth date, Social Security Number).
